All forms listed on this page should be downloaded and submitted to the front office prior to the first day of school

Checklist for required forms 2020-2021

MANDATORY FORMS

• Extended Day Registration Form
• Extended Day Parent Signature Page
  (All families must complete the  Extended Day registration. You will not be charged until or unless you use Extended Day the first time)
• Photo, Press, Audio, and Electronic Media Release for Minors
• Confidential Health History Update/Permission for Emergency Care
• Acceptable Use-Computer
• Handbook Agreement Form

MANDATORY HEALTH FORMS (only if they apply to your child)

• Food Allergy/Anaphylaxis Care Plan and Treatment Authorization
• Medication Authorization Form  (for over-the-counter and/or prescription) Read the Medication Letter for more information about medications to be given at school.
• Epinephrine Authorization
• Inhaler Authorization
• Updated Immunization Record to include Tdap (rising 7th graders)
